Youth / Adult Application

Please note: If you are already registered in another unit, you can either transfer or dual register (if you want to be registered in both a Troop, Crew, and/or Ship). If you were previously in Scouting, but are not current in your registration, please provide your past ID number when registering. This will help prevent some issues that may arise. Please contact us if you have any questions prior to completing the application.

The Annual Health and Medical Record must be completed annually and upon joining the unit. Visit the BSA website if you have any additional questions regarding the BSA Annual Health and Medical Form.

Instructions for Completing the Annual Health and Medical Record

1. Please make sure you are using the latest version of Adobe Reader.
2. Fill-in the form using Adobe Reader (handwritten forms will not be accepted), but do not sign it yet.
3. Save the form with a unique name before signing it (so that you can retrieve it later to edit).
4. Print the completed form.
5. Sign the completed form.
6. Turn-in the form to the next meeting/event.
